How GLP-1 use and eating disorders intersect

In a previous article for STAT in 2023, I highlighted the struggles caused by shortages of GLP-1 medications, leading to treatment delays and difficult decisions regarding who should receive these valuable drugs. The ethical dilemma of prescribing GLP-1s approved for diabetes for weight loss in non-diabetic individuals was a major concern at that time.

Fast forward to the present day, and a new clinical dilemma has emerged due to the excessive use of GLP-1 medications. Some patients are seeking to achieve an unhealthy and unsustainable body weight, posing a challenge for healthcare providers to address.

In my practice, I have observed that individuals who take GLP-1 therapies fall into different categories. The majority experience moderate weight loss of 10% to 20% over several months and are able to maintain most of that weight loss while continuing treatment. However, there are also non-responders who do not benefit from GLP-1 medications and super-responders who experience significant weight loss, sometimes exceeding 25%.

Determining the appropriate dosage of GLP-1 treatment can be a delicate balance, as patients often desire more weight loss even after reaching a plateau. It is crucial for healthcare providers to monitor for signs of excessive weight loss and potential eating disorders in patients undergoing GLP-1 therapy.

While GLP-1 medications can be life-changing for individuals struggling with cravings and overeating, it is important to recognize when weight loss becomes excessive and potentially harmful. Monitoring for side effects, such as muscle or bone loss, is essential in ensuring the safety and well-being of patients.

As a healthcare provider, I strive to emphasize the importance of overall health and well-being beyond just weight and BMI. Utilizing a combination of objective measurements and patient history, I assess whether a patient is at a healthy weight and make informed decisions regarding treatment.

In conclusion, while GLP-1 medications offer significant benefits for weight management, it is crucial to approach their use with caution and vigilance. Striking a balance between achieving weight loss goals and maintaining overall health is essential in providing optimal care for patients undergoing GLP-1 therapy.
